There are two ancient myths about the Tiber Island: according to the first one the island was formed when Tarquinius Superbus was thrown out of the city; the second tells that on board a ship which sailed from Rome to Epidavros in 291 b.C. there was a delegation which comprised some of the wisest men.

An important source, the Regesto farfense, gives information about the inauguration of the building and the name of its founder. The hospital was built in the tenth century by the will of the blessed countryside, a Roman nobleman who founded the monastery in honor of Saints Cosmas and Damian, two medical brothers called anargiri, that is, without money, because they treated the sick free of charge.
